Prosthetic
Limb Assistance Program
The
Fund reduces financial hardship on families by providing assistance
towards the cost of prostheses and artificial limbs for children and young
adults.
Eligibility
Requirements
To
be eligible, an applicant must satisfy the following requirements:

The
applicant must be a cancer survivor or currently diagnosed with
cancer.

The
applicant's need for the prosthetic limb(s) must be
cancer related.

The
applicant must complete the online Prosthetic
Limb Assistance Application.

A letter
from the applicant's physician supporting the cancer diagnosis and the
need for the prosthesis or artificial limb must
also be submitted. Medical information will be kept
confidential and will only be utilized to support the qualification of
consideration for the scholarship.

The
applicant must submit an essay discussing the
following question. HOW HAS MY EXPERIENCE WITH CANCER IMPACTED
MY LIFE VALUES AND CAREER GOALS? Essays must be a minimum of
500 words and a maximum of 1200 words.

The
applicants must agree
to do volunteer work to use their cancer experience to help other
young cancer patients and survivors coping with a life threatening or life-altering event.

The
applicants and their parents if they are minors, must sign and submit a release to agree to have their name and photo
published in the news media and
have their name, photo and success stories published on this
website.
